Registration Form

(Use this form to register for a LMA training seminar that you will be attending at the LMA Building.)

Instructions:  You Must Click on your desired topic/date.  To register for more than one, hold the Ctrl key down and click the ones you want.  Scroll down and type responses in the fields below and click on submit.  The reset button erases your responses so you can submit another person's information or correct an error.

These LMA Training Seminars are free and are held at 10:00am - 12:00 noon (except as noted), followed by light luncheon, in the First-Floor Meeting Rooms of the LMA Office Building at 700 North 10th Street. Click here for map.

DO NOT USE THIS REGISTRATION IF YOU PLAN ON ATTENDING BY WEBINAR. A separate e-mail invitation will be sent for the WEBINAR.
