Occupational License tax

As a service to our members who collect occupational license tax, the LMA has provided occupational license tax application forms, instruction booklets and tables.  In this area of our web site, we have included the latest copies of these items.  Select the item from the list below, and you can print it for reproduction.  

The following forms were developed and reviewed by the Occupational License Tax Committee of the Louisiana Association of Tax Administrators.  Click on the list below to access the PDF files for each.  You will need Adobe Acrobat Reader to print these files.  If you prefer, you can request these forms from the LMA office.
